STUDENT INFORMATION FORM PROCEDURES

Providing your child with the most appropriate classroom placement in order to maximize learning opportunities is valued at Hidden Valley. Parents are an important source of information and a part of this process. Parents have the opportunity to give the staff input about next year’s class placement using our “Student Information Forms.”

The Student Information Form is meant to provide information about the style of teaching that you believe would be most beneficial for your child. It is not meant for requesting a specific teacher. Your child’s current teacher and grade level team will have access to the form that you turn in. You are welcome to provide any positive comments about what has worked well for your child. Any forms with disparaging comments and/or specific requests for teachers will not be passed on for consideration.

These forms will be available in the office starting Monday, April 10th. They need to be returned to the office no later than Friday, April 21st to be considered.
